Postpartum Bleeding: Holistic Prevention Strategies

 

It is normal to have light bleeding in labor as your cervix dilates and breaks its tiny blood vessels. And as baby emerges from the birth canal there can some local tearing that can cause bleeding. Expect to experience the most bleeding at delivery and postpartum. Most of this bleeding is from where the placenta was located in your uterus.

At normal vaginal birth and immediate postpartum, it is common to lose up to a half liter of blood. After cesarean birth, one liter of blood loss is the average. After birth, your uterus needs to contract around the major blood vessels that supplied the placenta to close them off and prevent excessive bleeding. 

The first few days, bleeding can be like a heavy period. Then, it tapers to a moderate period, after which it becomes lighter and changes color over several weeks from shades of red, then pink to brown. The body is healing the former placental site, shedding the internal scab there, and extra tissue and blood that was lining your uterus during pregnancy. 

Postpartum hemorrhage usually occurs immediately, or up to the first 24 hours post birth, and remains a major cause of maternal death in the US and around the world. It must be taken seriously. Currently, there is substantial evidence in support of what is termed ‘active management of the third stage of labor,’ to reduce the risk of severe excess postpartum bleeding. It includes the use of:

  •  The synthetic hormone oxytocin (referred to as Pitocin in the US) via intravenous or intramuscular injection

  • Early cord clamping with waiting 1-3 minutes until baby gets at least most of the cord blood

  • Controlled traction on the cord along with counter pressure on the uterus to effect placenta delivery within the first 5-30 minutes after birth

  • Uterine massage to make sure it is firmly contracted

  • Assessments every 15 minutes for the first two hours.

The above process, or a similar version, is done routinely in most hospitals, and can certainly be done in out of hospital birth settings. However, the studies that determined these procedures, were based on hospital births in mostly resource poor but also well developed countries. Like all studies, they have their limitations and flaws, some were even considered to be of poor quality according to the esteemed Cochrane Review. Also these interventions are not without side effects and concerns. The American College of Nurse-Midwives support the use of active management of third stage of labor in low resource settings, according to their position statement, although they do admit its benefits are not as clear in the low risk healthy population, and encourage the provider to have a risk benefit discussion with each pregnant family so they can make an informed decision about it. 

Most homebirth and birth center moms and providers are passionate about physiologic birthing, minimal interventions and holistic modalities, do not routinely want an injection of medication, and are more interested in natural alternatives. They trust the incredible wisdom of the normal birthing process, which has worked for thousands of years or we would not have survived as a species. They share a common belief that if it is not broken, don’t fix it, wary of medication and interventions unless absolutely necessary and benefits outweigh risks. They tend to like the alternative, ‘expectant management’ approach, which also entails close observation by the provider, but tends to take longer, allowing for the normal physiologic process to take its course, and for interventions only if needed in select cases.

After birth, mom and baby are of course carefully assessed, but encouraged to bond skin to skin. There is no rush. Cord clamping is delayed until pulsation has ceased, or after placenta is birthed. Mom and baby are assisted to breastfeed which helps release mama’s own natural oxytocin.

The provider waits and watches for signs that the placenta is naturally separating and then assists mom into an optimal position usually using gravity, and encourages her to use her own bearing down efforts to birth her placenta. The provider may sometimes guide the birthing placenta with gentle traction on the cord, while supporting the uterus, then massages the uterus to make sure it is firm, assesses the bleeding until stable, and assesses and repairs tearing as needed.

Certainly, if there are certain concerns or risk factors, you may truly benefit from medical prevention and active management.

If there is an actual hemorrhage, make sure your provider is skilled, experienced, and fully equipped to deal with it with at least the commonly used effective medications, IV fluids, suturing material for lacerations needing repair, and hands on care that are usually sufficient to control it successfully. 

However, you can build up a strong blood supply and reduce excess bleeding and its risks with the following suggestions for natural support both in your pregnancy and postpartum.

Prenatal Support

Make sure you get checked and treated for anemia common in pregnancy, that your iron stores (ferritin) are sufficient.

Eat 3 large servings of wild greens or dark green leafy vegetables every day. They can be made into a salad, lightly sautéed or steamed. Good options are parsley, dandelion, alfalfa, kale, collard greens, comfrey and turnip greens.  For additional support, you can try the following:

NETTLE AND RASPBERRY TEA

Starting in the third trimester, drink 1 cup of this nourishing herbal infusion several times per day. 

  1. Combine a handful each of the dried herbs Nettles and Red raspberry leaf with 1 quart boiling water.

  2. Steep for at least 4 hours.

  3. Strain to a glass mason canning jar.

  4. You can add fresh mint leaves, lemon juice, or honey to taste.

GREEN DRINKS

Drink 1 ounce fresh, frozen, or powdered  wheatgrass juice 1-2 times daily to enrich and build your blood.

Or, try 1 scoop daily of powdered greens in your smoothie, 1-3 Tbsp bottled chlorophyll, or tablets or powders of spirulina and chlorella.

Postpartum

You need to rest in bed, on the couch or an outdoor lounge chair as much as possible for the first 2 weeks to recover.  Make sure you arrange for help in the home during this special time. Limiting activity and increasing rest help the area of open uterine blood vessels where the placenta detached to heal. 

Check the top of your uterus regularly for firmness, and massage it if it feels soft, until it hardens. Postpartum bleeding can be minimized when mothers are taught regular postpartum self massage of the uterus so that it stays firm and contracted around the blood vessels that supplied the placenta.

Start breastfeeding right away, and every 1 ½ - 3 hours thereafter, especially taking advantage of the times when your baby is awake and alert and eager to suck.  Nursing frequently causes the body to secrete its own natural hormone oxytocin to keep the uterus firm and decrease bleeding.

Urinate frequently to keep the bladder empty so the uterus can contract easier.

You can also take homeopathic caulophyllum 30 or 200 C immediately after delivery, then 3-4 pellets arnica 30C under your tongue every 2-3 hours. Or, try herbal shepherd's purse, 1 dropperful of the tincture three times daily for the first 3-5 days after birth. If you need additional herbal support for heavier or persistent bleeding, you can try a dropperful of Angelica tincture a few times daily.

If bleeding becomes heavier than a heavy period, and you are soaking through two maxi pads an hour for 2 hours, empty your bladder, make sure the top of your uterus is firm and massage it if soft until it becomes hard. If no relief, take 1 tsp shepherd’s purse herbal tincture under your tongue. You can repeat the dose a few times, but if the bleeding becomes heavier, contact your practitioner.

Lying-In and Postpartum Recovery with Earth Mama Organics

 

While having a new baby is truly an amazing experience, your new beginning with baby and your postpartum recovery should always begin with rest, healing and self care, also known as “Lying In”. Understanding that our bodies crave healing and support after the miracle of birth is incredibly important.

HAVE A PLAN. Make the most of the time you have with your new baby by creating a plan of recovery in advance. Postpartum struggles can be very overwhelming. By having a plan and sharing that plan with loved ones, you are eliminating some or all of the most common postpartum stressors. MEDITATE. Taking time to relax and calm your mind can make a huge difference in our external and internal energies, by sitting or lying in a comfortable position and focusing on your breathing; guiding it to be slow, deep and even (make the inhale to exhale ratio each to a count of 3 or 5 for example) or doubling the exhalation time (inhaling to a count of 3, exhaling to a count of 6) is especially soothing. If meditation isn’t your cup of tea, try to find time to relax in quiet with your favorite book or listen to calming, uplifting music or sounds. Know that it's alright to take a moment for yourself. Your energy and your mental health need nourishment during the postpartum period. 

PREPARE YOUR HOME/RECOVERY SUITE. When the ‘nesting phase’ kicks in, and it likely will, channel that energy into creating a space that is postpartum friendly and unique to you. In the weeks leading up to your labor, gather some or all of your postpartum/newborn necessities and make sure they are in a place that is easily accessible to you when you’re lying in with baby. The last thing you need is to run to the other side of your home to grab a diaper or nipple-balm as soon as you’ve laid down. Make sure your linens are clean and things are where you’d want them, so that you’re ready and able to spend your time healing and connecting with your baby.

Postpartum Struggles Are Real

 

Statistics show that about 15% of new mommas experience postpartum depression. And 70-80% of us experience “baby blues,” that 2 week period after the baby’s born when we can feel confused, overwhelmed, sensitive and just not ourselves.

If this is you, you’re obviously not alone. Here’s what you need to know: There is no failure in pregnancy, birth and postpartum.

You can move with grace through potential sadness, grief, fear, and anger to embrace and heal from your experience. When you are struggling with feeling overwhelmed and exhausted, depressed or anxious, unconfident and unsure, a holistic postpartum plan can give you back your joy and confidence. You got this. After My Own Last Birth I was Sick and Alone

Even as a nurse-midwife, I found myself in a postpartum situation that confused and scared me.

Several weeks after the baby was born, I began to feel like I was over caffeinated. My thoughts were racing, I was extremely agitated, panicked, completely overwhelmed and unable to function. Making decisions felt impossible and I couldn’t sleep. I felt faint, and was losing weight.

I was ashamed and embarrassed. How could I, an expert, find myself in such a state of confusion and intolerable symptoms of anxiety? I didn’t want to ask for help. The people I did ask were unable to treat me, and my husband was confused and worried.

I started to have scary and repetitive thoughts. Noises seemed too loud. Faces seemed too big. What the hell was wrong with me?! I felt like I was drugged. Somehow I suspected my thyroid was off, so I asked my colleague to draw my labs. Then I nearly passed out.

After a call to 911 and subsequent visits to a postpartum stress center, I learned that I had postpartum thyroiditis, an autoimmune condition in which the thyroid initially makes too much hormone, before not making enough.

The diagnosis gave me the confidence to finally accept help. I knew now that I wasn’t crazy. I knew I shouldn’t be embarrassed...but I was at the time. I stayed with my mother, who lived several hours away - near the center - for an entire month. And my husband and friends all worked together to care for my other kids at home.

I had to stop work for an entire year while recovering from my illness. And it took me a number of years to fully heal using a comprehensive holistic approach. My yoga and meditation practice were life changing, but the complete healing happened after intensive Clarity Breathwork sessions. It was the most delicious and miraculous feeling of relief I ever experienced.

The Forgotten Fourth Trimester

She was attended to by the community midwife, often the same person who was there for the births of all the other babies in the area and potentially even her own birth. After birth, she was off duty to rest, recover, and get the breastfeeding, while her tribe cooked, cleaned, took care of the other kids, and did needed errands.

Now, we live separated from our families, often experience pregnancy through postpartum more or less alone, and deliver our babies amongst strangers. A sister or mother-in-law might stay with us briefly after our babies are born, but then we’re on our own.

In the US, women are sent home alone after a few days in the hospital, and maternity leave is shockingly brief. We’re expected to pick up at work right where we left off after just a few short weeks at home with our new baby. Paternity leave is almost non-existent in this country, except for some more progressive family friendly workplaces.

Of course so many of us experience depression, anxiety, worry, and a lack of confidence! This way of viewing motherhood is simply unnatural. You can’t institutionalize the sacred.

But, thriving postpartum is possible. Especially when you go into it with a plan.

How to thrive postpartum

If you’re currently struggling in your “fourth trimester,” here are some tips you can implement right away to take the edge off.

Exercise. Try to make time for whatever type of movement feels good to you. Have someone take the baby while you go to a class. Put her in the stroller and go for a walk. Put on a workout video while she’s napping. Turn on some music that reflects your mood and move your emotions with your body; then turn on something upbeat and dance like noone is watching.

Connect with your community. Join a mommy support group either online or in-person. Reach out to friends and family that tend to cheer you up or make you feel comforted. Ask a neighbor to join you for a walk. Take a postpartum mommy and me yoga class. Don’t be afraid to open up about how you’re feeling. You might be surprised how many people can relate. And make sure to get daily soul nurturing hugs.

Meditate. Taking time to center down can make a big difference in our energy and confidence. Spend quiet time in nature. If you’re not accustomed to meditation, find a guided audio to listen to or just turn on some relaxing instrumental music, get comfortable and focus on your breath.

Nap. Now is not the time to stress about housework. Sleep deprivation leads to depression, anxiety, reduced memory and concentration, weight gain and a host of other health problems. Allow yourself to nap when the baby’s napping. The housework can wait.

Skip the sugar. Regularly eating sweets and simple carbs like breads and pastries puts us through a vicious cycle. Sugar crashes lead to low energy and low moods, which lead to carb cravings. Stick to fresh whole foods - and add more protein and healthy fat for a balanced diet and a balanced mood.

Do what you love. Each day, do things that bring you joy - even if it is for a few minutes at first. Think about what excites you and try to do more of it in your regular routine. Let it be easy and simple. That may be a quiet cup of tea, a visit with your friend, a stroll in the park, a flower or herbal bath in candlelight, watching comedy, getting a massage, doing some yoga. As baby gets older, make taking some time to yourself each day to do these things a non-negotiable like eating, drinking and sleeping.

Seek help. When your sadness or anxiety feels like more than “baby blues,” seek a qualified professional to help you through it. If, like me, you have an undiagnosed medical condition, the suggestions above just won’t be enough.

Postpartum Depression: Ending the Stigma

"The exact rate of postpartum depression is unknown because there are so many undiagnosed cases.  In the US, it’s estimated that up to 80% of women who give birth will experience some form of it, whether it’s the “baby blues” or a more severe type of postpartum depression (American Pregnancy Association). There are 4 million live births a year which means that 3.2 million women are at risk! This is before taking into account the women who have experienced stillbirths (about 24,000 happen each year in the US) and miscarriages (which happens between 660,000 and 880,000 times a year in the US) (CDC, Mayo Clinic).

As for clinical postpartum depression, specifically, about 900,000 cases are diagnosed each year (Postpartum Progress). Medical experts estimate that, when taking undiagnosed cases into account, that number could be twice as large!

Why are there so many undiagnosed cases? After conducting research and going through over 250 interviews (with moms, dads, and postpartum specialists) a team of students at Northwestern University speculate that the culture around postpartum lifestyle in the US may be a major factor.

When asked whose health came first (between the mom’s health or the baby’s health) the universal response of parents was the baby’s health. Moms think that the baby’s health comes first so much so that many of them don’t eat enough, rest enough, or practice enough self-care. Instead, they overexert themselves trying to obtain the “perfect motherhood” which many characterize with impossibly high standards in terms of the state of a mom’s baby, home, and over-all image.

Let’s start with the standards around the baby. It begins with the baby shower, where the mom receives stylish onesies, blankets, and tiny socks for her baby, who, not only is too young to enjoy its ‘gifts,’ but will quickly outgrow most of it. Those things then turn into more clutter for a mom to worry about.

Beyond the culture around the baby’s style, moms are most worried about their baby’s health. They also feel pressure to spend as much time as possible holding and interacting with her infant for the good of its development (an overwhelming message that is constantly pitched on parent blogs and other social media). Moms then feel incredibly guilty if her baby cries excessively, have issues feeding, or get sick because they see it as a sign of failure as a mom. In reality, these problems are normal and actually shared by most parents.

On Facebook, Pinterest, and Instagram, however, no one posts about these problems which make them seem uncommon and a sign of bad parenting. Parents on Facebook mostly post about the positives. Why would you want to update your friends, family, and coworkers about the fact that your infant isn’t taking to breastfeeding, or is causing you to suffer from sleep deprivation? It’s easier to post cute pictures of infants’ moments of joy and pretend that life is always blissful. Moms on Pinterest post about incredible DIY baby projects, intricate baby fashion, and picture perfect homes. They don’t post about the house getting messy when parenting gets super demanding, or the sweatpants they throw on when they don’t have time to choose a stylish new mom outfit (which is much more common). Instagram is where celebrity moms and fitness-bloggers-turned-moms post their rapid postpartum recovery: somehow they get a six pack a month after giving birth, and their makeup is always perfect. In reality, the postpartum recovery process takes a while and each body has different needs and fitness restrictions.

All of this culminates into unrealistic standards for motherhood. According to the interviews, many moms who looked back on past pregnancies admitted to buying into some of these standards and regretted the toll it took on their own well-being (some interviewees revealed that they had undiagnosed postpartum mental illnesses).

Not only do these standards play a role in damaging the well-being of many moms, but it also plays a role in creating a stigma around postpartum mental illnesses, by constantly plugging that motherhood is an overall joyful experience. Review of Bellefit Postpartum Girdle

 

I was gifted with a Bellefit postpartum girdle and invited to write a review. You know, I can only keep it real. SO...here is my opinion with some basic information to help you decide whether this is right for you. Overall I was impressed, but I do want add some important points and caveats.

Healthy women’s bodies recover intuitively and naturally on their own time after childbirth, without needing a support garment. Although a mama's body really never does look the same as it did before she was pregnant, even when she eventually loses her pregnancy weight (she looks better!), I encourage you to love and embrace your beautiful postpartum body and the normal postpartum changes that you feel both physically and emotionally. I invite you to bask in the gratitude of your experience, as there are countless women who would do just about anything to have such a gift that you are blessed with. Know that you are not alone, and like billions of women before you and along with you, you will continue to recover each day and eventually return your non-pregnant state, but now as a mom. Welcome to the tribe of motherhood! I have been there with you - four times.

I have a holistic approach to life, including healing after pregnancy and birthing. Nothing replaces abdominal toning and exercise for restoring muscle strength and tone - which I encourage for all mamas as soon as they feel up to it postpartum. Nothing replaces touch, slow deep abdominal breathing, and a 'love your postpartum body' perspective that I promote.  But I have found many mamas simply feel comforted by this support garment, especially early postpartum and temporarily as needed....to be used without forfeiting abdominal toning and strengthening exercise, breathing well and touch. I have found Bellefit supportive garments to help like they use belly binding around the world such as in Indonesia. They do aid in early postpartum healing and provide support many mamas feel comforted by. I deal with human beings and the reality is many postpartum mom's struggle with body image, feel frustrated that getting back to themselves takes longer than expected. Being into holistic health and healing includes being sensitive to real human struggles - the mind, body, heart and soul of each person and their unique situation. Having helped countless women with these issues after having a baby as a midwife, I have found many still love that binding and feel better with this support, and ability to fit into their pre-pregnancy clothes comfortably and sooner than they would if they went through a C-section or natural childbirth recovery without it - especially when they have to dress up and fit into a certain favorite outfit for a special occasion or wedding not long after having a baby. The Bellefit girdle is really a wonderful adjunct to your postpartum tool kit.

Bellefit is a mom owned brand, and was born out of a need to help women who need extra support after giving birth. The founder designed them to assist mothers like herself recover faster, help reduce pain and increase comfort.  Although many women are able to recover with ease, in addition to common discomforts and issues with changes in body image, there are many cases when a compression garment such as Bellefit helps a lot - like when women are faced with conditions such as Diastasis Recti, back pain or experience childbirth via C-Section, which is major abdominal surgery. It is the founder's hope to help provide this support to women who have experienced these challenging recovery journeys. 

The Bellefit girdles are recommended by many doctors and midwives, and have become so popular, even among celebrities - not that celebrity use matters to me.  The girdles are garments with Medical-grade compression and support that help in a variety of ways. They provide support for both your pendulous abdomen and back after pregnancy (especially helpful  after several babies), assist in releasing the increased fluids accumulated during pregnancy which reduces swelling, help relieve postpartum cramping, and enable you to move about, care for your baby and breastfeed more easily and comfortably. They also help to reduce incision pain after cesarean birth, and bring Diastasis Recti, the separation of abdominal muscles in pregnancy, back together. These benefits can result in feeling better emotionally, an important effect which is not to be taken lightly. When you are recovering well, you feel more at ease, more comfortable and have greater peace of mind, and can better enjoy this precious time of your life and bonding with your new baby.

You need to wear the girdle tightly around your torso, to maximize its benefits, following the birth of your baby.  The girdle should fit quite snugly around the midsection, which applies postpartum abdominal compression. It is this tightness and steady pressure around a your abdomen and lumbar region that helps aid in recovery and support after baby is born. It is this tightness and pressure that is even alleged to help straighten your spine and improve posture; but I have found osteopathic or chiropractic adjustment, and ongoing alignment based, strengthening yoga practice would be more effective modalities for this. The girdle is said to help your organs displaced in pregnancy go back to where they were a bit faster, but this will happen normally on its own with a dose of patience. The Bellefit girdle supposedly helps with lymphatic drainage and removal of toxins but I have not found evidence for this; and it allegedly compresses and decreases enlargement of fat cells, but really, the most effective way to lose excess weight and get back into shape is healthy eating and regular exercise.

Many women report liking its corset style, feeling that it is more comfortable than imagined. It has convenient flap with hooks in your groin area, and it so well made that it does not roll down your back or roll up your buttocks. This fabric itself feels nice and soft on your skin and the clips hold together very well. It can look and feel quite sexy by itself. and it can be worn under clothing without being visible. It does seem to run small so make sure you get the size that is best for you. It's tight fit may take some getting used to, but overall mamas come to really like it.

It is available in a variety of styles - duel closure, corset, girdle zipper, pull up - ranging in price from $72.50 to $145, and Bellefit sells other garments for support during pregnancy. I do like the garment for support and alleviating back and low pelvic discomfort during pregnancy as well - I have also found it effective to keep breech babies turned vertex, stay vertex, and help like a Rebozzo in long non progressing labors, assist baby's head to be better applied to the cervix for uterine contractions to more effectively result in dilatation and descent - especially mamas with lax abdominal tone and pendulous abdomens, after several pregnancies without toning in between. Bellefit has great customer service. They will help you with sizing and answer your questions promptly, which is why it is ideal to order directly from them rather than Amazon for example.
